Vaginismo
Sei sola nella tua stanza e ti sorge l'angoscia all'idea di prenotare una visita ginecologica, al pensare che il tuo fidanzato possa chiederti un rapporto “completo”, persino nell'immaginare che sia tu, con le tue mani ed i tuoi tempi, ad avvicinarti alla tua vagina.
Parlarne a qualcuno è difficile, hai paura del giudizio del tuo medico di base, del tuo compagno, della tua migliore amica. Alla vergogna preferisci il silenzio, lo sfuggire alle situazioni “pericolose”, ti consideri sbagliata, anormale, fuori posto, ma non sai che nome ha il tuo problema, a chi rivolgerti per aiuto. Ciò che ti fa soffrire si chiama vaginismo, e ne soffre una donna su cento.
Dare un nome al tuo disagio è il primo passo, il secondo è sapere che non sei sola, il terzo è capire chi contattare e cosa fare per poter guarire.
Partiamo dalla definizione scientifica del vaginismo:
“Il vaginismo è la contrazione involontaria dei muscoli circostanti l’ingresso vaginale nelle donne che non presentano anomalie negli organi genitali. La forte contrazione muscolare rende doloroso o impossibile il rapporto sessuale, o qualsiasi attività sessuale che comporti penetrazione.”
Il vaginismo ha varie gradazioni, che vanno dal forte dolore provato durante la penetrazione fino all'angoscia e alla totale contrazione della vulva alla sola idea di una visita ginecologica o di applicare l'assorbente. Il perché questo avviene è dovuto all'azione di difesa del cervello primario, la parte più antica della nostra struttura celebrale. Il cervello primario si attiva quando percepisce situazioni in cui la nostra vita è in pericolo e la nostra integrità fisica può essere compromessa, facendo scattare un senso d'angoscia a livello psicologico e la contrazione dei muscoli a livello fisico. In chi soffre di vaginismo il cervello primario scatta quando qualcosa si avvicina entra o si avvicina alla vagina, talvolta persino solo all'idea che questo accada, creando angoscia e contrazione dei muscoli pelvici. E' quindi un problema fisico e psicologico, che va affrontato da un professionista che ti aiuti a superarlo.
Per farlo come professionista io utilizzo una terapia basata sul metodo EMDR e ipnosi medica. L'EMDR (acronimo di Eye Movement Desensitization and Reprocessing) è un approccio terapeutico che consiste nell'aiutare ad elaborare gli eventi insieme alle emozioni, percezioni, cognizioni e sensazioni fisiche disturbanti che hanno caratterizzato il vaginismo. Con l’EMDR è dunque possibile alleviare la sofferenza emotiva, permettere la riformulazione delle credenze negative e ridurre l’iperattivazione fisiologica del paziente.
Questo approccio risulta efficace anche con i pazienti che posso aver difficoltà a verbalizzare possibili eventi traumatici che abbiano vissuto.
L'ipnosi medica è una procedura in cui il paziente condotto dallo psicoterapeuta sperimenta cambiamenti a livello di sensazioni, percezioni, pensieri e comportamento. Il tutto ha inizio tramite una procedura riformulazione del proprio stato di malessere in uno di rilassamento, benessere e calma vigile.
Quando utile, alla terapia psicologica potrò consigliarti di accompagnare un supporto dell’ostetrica per la parte riabilitativa.
